Provides structured, goal-oriented casework for families in the Permanent Supportive Housing Program. This position will also include intake, eligibility and needs assessment for disabled, homeless and chronically homeless individuals and families seeking to enter the Shelter Plus Care Program. Coordination of services with mental health, behavioral health, chemical dependency treatment and other social service programs and providers.
This position has a social justice component that requires critical thinking through the lens of racism and intersections with poverty. Knowing the core principals of antiracism and grounding those principles in everyday work are required job skills and core values.
